Diabetes UK Scotland is committed to patient centred care and for people with diabetes to be able to manage the condition and enhance their quality of life.  The charity has received funding from the Scottish Government to maintain existing pilot areas, extend the service to a further 3 NHS Health Board Areas and work towards a sustainable model of peer support for people with diabetes across Scotland.

The Diabetes Action Plan 2010 – Quality Care for Diabetes in Scotland highlights the need for a range of emotional, social and psychological support services to help people with diabetes to manage their condition. The Peer Support Project will  review the current  provision of peer support for people with diabetes in Scotland, identify a sustainable model of quality peer support for people with diabetes across 6  sites in Scotland, evaluate the strengths and weaknesses of the Peer Support Project, share learning from the project with the Scottish Diabetes Group, and make recommendations  for a roll out of peer support across Scotland for the benefits of people living with diabetes, their families and healthcare professionals.

We need an experienced and creative Project Co-ordinator to take this work forward.  The Project Co-ordinator will support the 3 existing catchment areas, extend the service to 3 additional sites, communicate with key stakeholders of the project, be responsible for the project planning and will report to the Patient and Health Care Professional Engagement Manager.

You must have experience in developing, managing and delivering peer support services, communicating with a wide range of stakeholders, and working to tight deadlines. You will be able to manage competing priorities, be competent with IT applications and be comfortable presenting to a range of stakeholders.

You must possess a full driving licence.

The post involves occasional weekend and evening work and is offered as a full time fixed term post.  Secondments will be considered.
