By sending us your story, you agree that we may publish it on our website, in our printed magazines and in some cases, our campaign material.

We regret we cannot respond to every story submitted.

If you are not happy for your first name to be published, please let us know by selecting 'no' from the drop down menu below. In some cases, we may contact you to ask for any additional details, or to talk about any changes we need to make.

There may be occasions when the general media may be interested in 'Your story' or interviewing you about your diabetes. Further down the form, you are given the chance to say  whether you would be happy to discuss this with the wider media.

No information will be passed onto the media without your permission and we would always contact you first before giving your details to the media.
