What if I can’t get to the clinic?

Some care homes will expect you to make your own arrangements to get to your annual review. This may require arranging for friends or family to transport you or paying for a taxi.

If you are unable to attend the clinic because of phyical fraility, the staff will make arrangements for somone to visit the home to carry out your review.

Your annual review is extremely important for your overall good management of the condition, so do either keep a check on when you should be seen, or ask someone to be responsible for doing it for you.

If there are problems making arrangements, contact:

  • the practice manager at your doctor’s surgery
  • your local Patient and Advice Liaison service (PALS). This is usually based in your local hospital, so the switchboard would be able to give you the correct number.
