Food and nutrition in care homes - individualised information

The vital role of the registered dietitian in the nutritional therapy of the person with diabetes as well as the older person in a care home should not be underestimated. 

Care homes should seek to establish good working relationships with the dietetic service and work together to ensure effective nutritional therapy for those residents with diabetes. 

Every person with diabetes should have an individualised nutritional care plan in place that has been discussed and agreed with the person with diabetes and their family/carers where appropriate. The dietitian is best placed to facilitate this part of the care planning process.
