Dischage and follow-up plans are an important mechanism for ensuring continuity of care, appropriate onward referral and effective communication with the care home staff and day-to-day healthcare team.
You should be given information about the outcomes of your hospital stay and any ongoing follow up.
You should be given written information about how to continue managing your diabetes, especially if any changes have been made to your treatment.
You should be given a copy of your discharge letter, which you will be able to give to your care home staff.
Your GP should receive a copy of your discharge letter within a week of your discharge.
