It is important that each person has an individualised care plan so that all staff are able to refer to it and update it as necessary.
The care plan not only identifies who is responsible for each person’s diabetes care, but also can give details of:
- Treatment
- Dietary requirements
- Activity levels
- Contact numbers
- Targets for blood glucose, blood pressure and other tests.
It should be a document prepared in agreement with the person themselves, their family and staff. Other specialists, such as occupational therapists, physiotherapists, dietitians or psychologists, may add to it.
The annual diabetes review information can be contained on it and, should hospital admission be necessary, a copy can be sent with the person to assist the staff there.
