Identity card for insulin users

An insulin user’s identity card is useful when you are travelling and carrying insulin, and also when you go to clubs or pubs.

This identity card carries emergency contact details and treatment information.

It also carries your photo and the phrase “I have diabetes treated with insulin” in English, French, German, Spanish and Arabic.

You can order one for £5 by calling the Diabetes UK Supporter Services team on 0845 123 2399 for an application form or emailing supporterservices@diabetes.org.uk - or simply download the form from our online shop.
