The Year of Care is a new way of providing care to people with long-term conditions. It is currently being tested in Calderdale & Kirklees, North of Tyne and Tower Hamlets.
The Year of Care is about:
- Making routine consultations between clinicians and people with long term conditions truly collaborative through care planning.
- Using reliable information from care planning about individual choices and needs to directly inform the development of the services available locally through commissioning.
