It is an opportunity to check that your care plan is what you want and need. Your care plan should be developed and agreed based on what you can do and what your diabetes healthcare team will do to help you achieve your goals.

The 15 healthcare essentials checklist gives details of the important aspects of diabetes care that should be checked at least once a year, but may be done over a series of appointments and in different places. The results of all tests and examinations should be discussed with your healthcare team, during your appointment. You should receive copies of any letters written about you, if you wish, and have the opportunity to discuss your results and what they mean during your care planning review.
