Completing your application form
Please look carefully through the list of events and select the most suitable for you. Please complete the form in full and give all the details we request. Some of the information may be repeated but it is necessary for staff when planning the weekend. It is important to keep us up to date with any changes in your address and telephone numbers during the period before the event.
Priorities and alternatives
The Family Events are allocated first to those who have not attended before. The events are very popular and if your first choice is fully booked you will be offered an alternative where available.
Confirmation
On receipt of your application form, the information will be entered onto our database and if places are available, you will receive a confirmation of your place.
Payment
When we send confirmation of your place, we will also send an invoice requesting payment, either to you or the organisation paying for the event. If you are using payment methods 2 or 3 we will contact the named person to confirm that funds will be granted to pay for the event. Diabetes UK has a limited amount of funds to help pay for families who could not otherwise attend. We will help you to access this fund if necessary so that the cost does not prevent you from attending.
Please do not send any money before confirmation. The full cost should be paid no later than one month before the start of the event and can be paid in instalments.
All meals, children’s snacks, activities and trips out will be included in the price. Bar bills, telephone calls and newspapers, etc., are not included in the price.
Further information
Approximately three weeks before the event takes place you will be sent further information including programmes, route map and staff lists.
Family Events are very popular and we want to be able to offer places to all families who apply. Once you have received confirmation, we will reserve a place for your family in the hotel and the organisers will plan the activities to include your child(ren).
Cancellation
If you cancel your place, an administration charge of £25 will be invoiced to you. If you cancel 28 days or less before the weekend takes place and we are unable to fill your place with another family, you will be asked to meet the full cost of the event. Diabetes UK will be charged the full cost for your family even if you do not attend.
Data protection
The Data Protection Act (1998) has come into effect. Diabetes UK can confirm that your completed application form will be treated confidentially and will only be distributed to the Event Organiser, children’s coordinator and senior medical team upon confirmation of your place on an event and your agreement for us to do so.
