Hardwick CCG in North East Derbyshire has a relatively high rate of prevalence of Diabetes and challenges in terms of improving performance against key outcomes areas. They are aiming to help people with diabetes to improve the self-management of their condition as well as encourage early diagnosis of diabetes by increasing take-up of NHS Health Checks.

Diabetes UK will be joint-hosting a series of focus groups with Hardwick CCG to engage local people with diabetes. The focus groups will be held across the patch covered by the CCG, and will give service users the opportunity to tell them their priorities for improving diabetes care. This will be used to shape the focus of the CCG’s service improvement projects. We will then work together to establish an ongoing user reference group which influence the service improvements as they are implemented.
