North Hampshire CCG have identified improving outcomes in diabetes care as a key priority. As such they are in the process of redesigning the pathway for people living with diabetes to ensure that patients are able to access the majority of services within community based settings, and that services meet NICE quality standards.

In December the CCG joint-hosted an event with Diabetes UK for around 40 local people with diabetes. From this the CCG gained valuable information on people’s experiences of local diabetes services, and their views on the proposed pathway which will influence the way services are re-designed. In the coming months we will be undertaking some focused one-off pieces of work with people with diabetes around Care Planning and the diabetes-related information available on the CCG website. Diabetes UK will then assist the CCG in recruiting a patient representative to their local Diabetes Network.
