Report from the joint DH and Diabetes UK Care Planning Working Group

A joint report has been produced by the Department of Health (DH) and Diabetes UK to offer guidance on the care planning process for diabetes.
Care planning offers people with diabetes active involvement in deciding, agreeing and owning how their diabetes will be managed through partnership with healthcare professionals.
A joint working group produced the report, which explains the principles and processes of care planning, reviews the evidence base, and offers guidance on putting it into practice.
"There is a real push to get people with diabetes involved in shaping their services," said Simon O'Neill, Director of Care at Diabetes UK.
"Involving patients in discussions about their care on an equal level will ensure choice in services."
