Summary:
A programme was set up to identify people with diabetes who were not yet diagnosed, which would increase awareness of diabetes, identify high risk risk groups for screening, increase the capacity to perform oral glucose tolerance tests and improve skills in primary care in screening and accurate diagnosis. The programme also offers structured education to people who are newly diagnosed.
By working as an integrated team, primary and specialist health care professionals have been able to achieve a common goal leading to improved networking and better relationships between all partners in the programme.

Summary:
To ensure effective and good quality care is provided for people with diabetes when they are admitted to hospital. Services provided including daily ward round by band 7 diabetes specialist nurse to triage all known individuals with diabetes and as risk individuals (e.g. individuals on steroids or Percutaneous Endoscopic Gastrostomy feed); also to review and refer on when appropriate.

Summary:
The aim of the project is to detect people with undiagnosed Type 2 diabetes through a screening service and involve engaging community pharmacists to deliver effective care for people with diabetes.
The team also provide a medicines use review to people with diabetes. This is to address any medicines management issues arise in the use and understanding of the medication that they take and to address the problem of compliance to therapy.
