Specialist Knowledge about Carbohydrate Counting for Self-Management (SNACCS)

Summary:

The Diabetes Centre within the North of Tyne Primary Care Trust has developed a course to support people with Type 1 diabetes to better manage their condition.  It is designed for those who want to use a flexible insulin regime along with carbohydrate counting to further develop their self-management skills allowing optimisation of their glycaemic control.

Diabetes and pregnancy multi-language DVD: Educational tool

Summary:

This project developed a training tool to increase awareness of the need for good pre-conception advice for women with diabetes.  A DVD has been developed for use by healthcare professionals and patients; aimed at all women with Type 2 diabetes of child bearing age, their partners and healthcare teams. It raises awareness of problems with poor diabetes control before pregnancy in order to improve the pre-conception care provided.

X-PERT Programme Structured Education for people with Diabetes

Summary:

Urdu speaking South Asian Community


This is a structured education programme devised for people with Type 2 diabetes, it has also been developed for the Urdu speaking South Asian community. Based on patient activation, discovery learning and empowerment. The programme has been evaluated using a randomised control trial methodology. It meets all the quality assurance standards for a structured education programme.

Diabetes Education through Adult learning (DEAL) – NOV 08

Summary:

The team offer a whole suite of education programmes that, in line with current national guidance, cater for all of the diabetes education needs of the local population, using an adult learning theory – centred approach that engages people with diabetes.

A Group Education programme for South Asians with Type 2 Diabetes – Aug 08

Summary:

The aim of the programme was to enable South Asian adults with type 2 diabetes and able to understand Punjabi or Hindi dialect to increase their self management skills, knowledge and confidence in managing their diabetes.  This is a structured education for people with diabetes and run over 5 once weekly sessions adapted from the X-Pert course but delivered in Punjabi and Hindi. The programme is aiming for South Asian adult with diabetes who has been diagnosed with type 2 diabetes and their carers/ partners and is held in a community setting e.g. temples.

BERTIE in Essex - Aug 2008

Summary:

The programme is a 4 day structured education course over 4 consecutive weeks for people with type I diabetes, with follow up sessions at 3, 6, 12 and 24 months.  There are 4 trained educators and the programme is based on the BERTIE course.

ASPIRE: Structured education for adults with Type 1 Diabetes- July 2008

Summary: ASPIRE is a 4 week course run by members of the Chesterfield Royal Hospital Diabetes Team. It involves attendance one day each week between 9.00am and 4.00pm and also involves various aspects of diabetes management and includes intensive self monitoring of blood glucose and food intake.

Brent Ramadan Diabetes Taskforce - READ programme (Ramadan focused Education and Awareness in Diabetes) July 2008

Summary:

In Brent, a Ramadan focused structured education programme is delivered by a team of doctors, dieticians, nurses and is taught in groups of 15 maximum. Topics covered including nutrition, meal planning, hydration, physical activity, medications, when to stop fasting. They also teach the diabetes related principles such as blood glucose monitoring and recognising complications e.g. dehydration and hypoglycaemia.

Store Tour for People with Diabetes April 2008

Summary:

The service offer people with diabetes access to dietary advice from a dietitian within 4 weeks of being referred to the service.

Torbay Insulin and Food Adjustment course (TIFA)<br>September 2006

Summary:

The aim of the TIFA course is to educate, in a small group, people with Type 1 diabetes. There are general educational components focussing on complications, illness, travel and life style, but the main focus is on carbohydrate counting and the matching of insulin dose to food intake and activity.

The programme runs for 1 two hourly, and four, four hourly sessions held once weekly over consecutive weeks. At six weeks the participants come back as a group to discuss any areas that may have been a success, and share these experiences with the group, and/ or discuss areas that might be causing them a problem. They are invited again at three months for the same and to meet the participants of the current programme running.

Annually, at a hotel all previous participants of the year are invited to a refresher session with relatives and friends.

Managing Diabetes - Making Choices (previously The Diabetes Look After Yourself (LAY) Course<br>December 2005

Summary: This project's aims are: Promoting a positive lifestyle; Enabling individuals to achieve what is appropriate for them; Starting from each person?s values, perceptions and priorities; Focusing on encouraging independent skills to develop self-confidence, motivation and ability to control/influence health promoting decisions.

The teaching programme is flexible so that emphasis can be placed on group needs and priorities. The quantitative and qualitative results showed that the course can empower individuals with Type 2 to develop their own strategies for self management.

The course utilises a variety of teaching strategies designed to improve psychological well-being and promote self-efficacy. Group support from others who have diabetes is emphasised and participants are encouraged to share their feelings with group members and the ways they have coped.

Summary: The Type 1 Diabetes Workbook developed by the Type 1 Network is an example of a resource for people with Type 1 Diabetes intended to accompany their diabetes learning programme.

Dose Adjustment for Normal Eating (DAFNE)<br>November 2005

Summary: This programme is a structured education programme for people with Type 1 diabetes. The underlying principle of DAFNE is that insulin doses should be adjusted to fit in with the food a person chooses to eat at the time they choose to eat it.
The programme currently meets all the NICE criteria for structured education programmes for people with diabetes.

Dietetic Services In Birmingham<br> November 2005

Summary:

Black and Minority Ethnic Communities


This programme aims to address the educational needs of black and minority ethnic groups in the Birmingham area.
People are triaged and according to need offered a group or one-to-one education intervention.
Sessions are available, in appropriate languages with the use of interpreters. Sessions are run in community settings and friends and family can attend.
Evaluation is ongoing and quality assurance is being worked towards.

Summary: A structured group education programme for people newly diagnosed with Type 2 diabetes. The programme has a sound theoretical and philosophical basis designed to empower people to self manage their own diabetes.

One-day community-based programme: Getting started with Type 2 diabetes (updated in April 2008)

Summary: The programme is run for people with newly diagnosed Type 2 diabetes in Portsmouth City. It is run in community centres to reduce the emphasis on medical aspects and improve accessibility for people. The philosophy of the programme emphasises psychological theory, empowerment, patient interaction and involvement and takes a person centred approach. Participants are encouraged to choose a priority area for lifestyle change, and then set themselves goals to achieve this whilst anticipating any barriers

Diabetes storytelling project <br>December 2002 progress update 2006

Summary:

Bangladeshi Community


This project arose out of initial research, funded by the Wellcome Trust that showed that positive changes in health behaviour among Bangladeshi people with diabetes tended to be associated with stories told by another member of the community rather than information from health professionals.

This project has contributed to improving health by: increasing knowledge and understanding of diabetes for Bangladeshi patients, NHS and community bi-lingual health workers
