Summary:
A website has been developed to improve care planning where healthcare professionals and people with diabetes have access to the same information ensuring consistency across primary and secondary care. Focus is on self-management with the person with diabetes driving things forward. A hand held record has been produced which compliments the electronic information available online.

Summary:
The Diabetes Personal Record is owned by the people with diabetes and presented to hospital and community staff for completion at attendance when necessary. It includes information on the individual’s diagnosis, treatment and support groups. The record has been used to improve communication between and within levels of care, and to promote user’s involvement in their own diabetes care.

Summary: This project aims to develop a user-friendly, comprehensive prototype diabetes record book or databank with input from a representative cross-section of health-care workers and patients. Feedback will be used from the patients to modify the prototype and produce a databank for subsequent larger scale patient and health care worker assessment.
