Call for Entries
Diabetes UK Shared Practice Database shares its database of diabetes services to support local service development. As part of the shared practice work, Diabetes UK has developed an award to encourage projects intended to improve the health and quality of life for people with diabetes.
The Shared Practice Award is a new scheme to recognise achievement and excellence in diabetes care to support shared learning. Winners and finalists will enjoy:
High profile recognition from local communities, service users and partners
An opportunity to share practice with peers
The winning project team will receive up to 3 free places to attend Diabetes UK Annual Professional Conference (APC) 2010
Moreover, all applicants will find applying for the Shared Practice Award:

Eligibility Criteria
Open to all including those not already on Shared Practice Database
The submission of an entry indicates the submitters agreement for their project to be publicized on Shared Practice Database
Time frame for when the project was conducted is open, but the impact should have occurred recently
Projects can be replicated by other organisations or that others could learn from
Selection Criteria
Each project will be evaluated on:
Levels of users involvement
Learning has been demonstrated, identified and implemented
The Impact of the project
How Person centred is the project
Sustainability and transferability; and
Degree of Innovation
Application Guidance
Applications must be submitted online or by mail to the Diabetes UK by Friday, 7 November 2008. The prize winner will be announced in March 2009 at the Annual Professional Conference (APC) in Glasgow. All examples will be assessed by judges anonymously and there will be a panel of 6 judges including user representatives.
