Diabetes UK Shared Practice Award for Person Centred Care 2009                                                                    

Your application must include:   

Part 1: Summary describing the project (word limit of 1500 words overall):   

Each project will be evaluated on:

1. Level of user's involvement - please detail your process for involvement, service user's roles and responsibilities.  

2. Learning has been demonstrated, identified and implemented - please describe what shared learning has arisen from the project and how this has been implemented.  

3. The impact of project - please include data and feedback to support this.  

4. How person-centred is the project - please describe how the services has tailored to individual need.  

5. Sustainability and transferability - what further development is planned or in progress; and  

6. Service innovations - please include any innovative approach to the provision or support of health care    

Part 2: Contact information for the following individuals:  

1. Nominator/ Lead Researcher will be contacted:

·        To confirm the application has been received

·        To supply any missing information

·        When the judging panel has identified the winner

·        To work with the individual and disseminate the project information

 

Only completed applications that comply with the instructions above will be reviewed.  Applications must be received by Friday, 7 November, 2008.
