Please select a suggested donation amount or give whatever you can by entering your own chosen amount.
I am a UK tax payer and want Diabetes UK to treat the donations I have made in the past four years and all future donations as Gift Aid donations. I understand that my donations are eligible for the scheme if I pay enough UK Income and/or Capital Gains Tax to cover the amount of tax that all charities I donate to will reclaim in that tax year. If I pay less it is my responsibility to pay back the difference.
Please notify us if you change your name, address or no longer pay sufficient tax.Your full name and address are needed to identify you as a current UK taxpayer.
Additional informationIf you pay Income Tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your additional Gift Aid donations on your self-assessment tax return or ask HM Revenue and Customs to adjust your tax code.
I do not want Diabetes UK to claim Gift Aid on my donation.
