Order patient information packs - North of England

Diabetes UK's Patient Information Packs (PIP) are available to Clinical Commissioning Groups (CCGs) to buy and distribute to their patients. A PIP is aimed at newly diagnosed patients with type 2 diabetes, but may also be of benefit to some people already living with type 2 diabetes.

The packs provide important information and signposting to help people confidently self-manage their diabetes, something which we know has been particularly important during the pandemic. Information in the packs also signposts people to key local NHS services, as well as the Diabetes UK Learning Zone and Helpline.

The local information content can be tailored to support local priorities, such as encouraging uptake of local diabetes education courses, access to footcare services or retinal screening etc. Because they are a print resource, they may also be useful in tackling some elements of digital exclusion.

A PIP contains the following:

  • 1 x Single pocket folder (no poly bag)
  • 1 x Personalised CCG letter
  • 1 x Everyday life with type 2 diabetes guide
  • 1 x Healthcare Essentials leaflet (new version)
  • 1 x Learning Zone postcard/flyer
  • 2-3 optional inserts from the CCG about local service

This is a commissioned resource, so there is a cost associated. We are happy to work with you to provide a bespoke quote, depending on your requirements.
