Listening exercise on care planning

Please take part in an NHS Listening exercise on care planning by downloading and completing the form on the right, and returning it to the National Diabetes Support Team.

Care planning is one of the key interventions required to achieve Standard 3 of the Diabetes National Service Framework. Care planning is the process which offers people active involvement in deciding, agreeing and owning how their diabetes will be managed. It is underpinned by the principles of patient-centredness and partnership.

The joint Department of Health and Diabetes UK Care Planning Working Group, published their report ‘Care Planning in Diabetes’ in December 2006. The complete publication is available at

Your PCTs should know their local results from the Healthcare Commission survey 'The views of people with diabetes' and the Improvement Review. If your PCT scored fair or below locally on care planning and self-management, work will be needed to address this.
