Help develop ‘Year of Care’ for people with diabetes

A new pilot scheme aimed at ‘planned care’ for people with diabetes is being launched.

The Year of Care for diabetes aims to develop a patient’s annual review into a ‘care planning’ discussion. The hope is to give patients more of a say about the care they receive over a year, including self-management support.  

Diabetes UK, the Department of Health and the NHS National Diabetes Support Team, are working in partnership to produce this project.

This new scheme is looking for applications from organisations involved in commissioning in England, such as Primary Care Trusts (PCTs), Diabetes Networks or consortia of Practice-Based Commissioners to test how the local NHS can put the approach into practice.  

Bridget Turner, Head of Healthcare and Policy at Diabetes UK, said: "This is an exciting initiative which will help us understand much more about how local health services can provide care and support to meet the individual needs of people with diabetes.
