Free Diabetes UK 'Diabetes Care and You' booklets for GPs and their patients

Diabetes UK, in partnership with NHS Diabetes, has produced 'Diabetes Care and You' – a booklet to provide key information to people with diabetes about their role in managing their diabetes, working together with their healthcare team, and what they should expect from the NHS.

The booklet aims to help people to work in partnership with their diabetes care team to agree individual goals and the support needed to achieve them – through care planning, so that they can manage their diabetes effectively and lead a healthy and normal life.

It includes a handy pullout care review checklist which details the elements of diabetes care that should be checked and discussed with the diabetes healthcare team at least once a year.

Endorsed by NHS Diabetes

NHS Diabetes is endorsing the booklet and recommends that GPs use it when they meet with patients to trigger discussions about their diabetes, and to discuss and agree the care they need to support self-management. The booklet will help GPs to work with people with diabetes to achieve QOF targets. (QOF stands for Quality and Outcomes Framework, the system for performance management and payment of GPs.)

Diabetes UK gives 30 copies to every GP

Diabetes UK will be sending 30 copies of the booklet and a set of vouchers to each GP surgery in England in mid-November to coincide with World Diabetes Day.

Vouchers for free magazine for patients

The vouchers, which are for GPs to give to people newly diagnosed with diabetes, entitle them to a free copy of the Diabetes UK magazine 'Diabetes for Beginners'.

Improving knowledge and involvement

“Evidence shows that many people with diabetes are not involved in discussions about their own care and that only 10 per cent have attended a structured education course to learn more about diabetes management," said Professor Sir George Alberti, Chair of Diabetes UK.

"It is therefore crucial that we do all we can to improve people’s knowledge of diabetes and we believe that this new booklet will be a handy tool for both GPs and their patients.”

A booklet everyone with diabetes should read

Dr Rowan Hillson MBE, National Clinical Director for Diabetes said "The person with diabetes is the most important member of the diabetes team. It is really important that people with diabetes learn as much as they can about their condition, how it may affect them, and what they can do to ensure that they keep as well as possible. 'Diabetes Care and You' is a friendly booklet, full of good information, and I suggest everyone with diabetes reads it – even if you have had diabetes for a long time."
