How do you use information prescriptions?

We want to know how you use information prescriptions

In January 2015, Diabetes UK launched a series of information prescriptions. Information prescriptions are designed to help clinicians and patients make decisions together about the treatment and self-management of high blood pressure, high HbA1c, and bad cholesterol. They are available on Primary Care IT systems.

If you have used information prescriptions, please complete this 15-minutesurvey to let us know your views on them.

Who should complete this questionnaire?

Any GPs, doctors, nurses, or healthcare assistants that have used the Diabetes UK information prescriptions at least once.

What is the survey for?

We are working with an independent researcher at Newcastle University to understand the impact of information prescribing on professional behaviour. We want to assess your views on how to use the Diabetes UK information prescriptions with your patients with diabetes.
