Information prescriptions are a tool for healthcare professionals and people with diabetes to work together to understand and improve on their health targets. Watch one of our Primary Care Network members talk about our new tool and how it is changing consultations.

 

Information Prescriptions are a short (single side of A4), easy to read and clinically accurate resource that helps healthcare professionals and people with diabetes to make decisions together about the treatment and self-management of their health targets. 

They include actions to reduce the risk of complications, and a section for setting individualised behavioural change goals, needed to meet these health targets.

Diabetes UK are currently piloting resources to help practitioners talk to patients about their risk of Type 2 diabetes. Our Prevention Postcards are a simple way for healthcare professionals to signpost those at risk of Type 2 diabetes straight to information and support about the realities of having the condition. The booklet (PDF 194 Kb) patients will receive explains what Type 2 diabetes is and makes clear the risk factors. It also contains advice on how to make simple, lasting changes to reduce their risk.

If you feel your patients may require further information please signpost them to theDiabetes UK Helpline. The Careline can provide confidential support on diabetes related issues. All team members are trained and experienced counsellors.

Diabetes UK has created a simple way for healthcare professionals to signpost people with diabetes straight to information and support around how to eat well with diabetes. OurEating Well Postcardsallow you to give your patients reliable and trustworthy dietary information without having to carry the information around with you from practice to practice. The Eating Well Postcards are designed for healthcare professionals to hand to their patients, who can fill out the card and freepost it back to Diabetes UK. We will then send them anEnjoy Food  pack which has all the information and tools for people to eat better and manage their diabetes.

Diabetes UK has created an innovative way of reaching the people at highest risk of devastating diabetes complications with an evidence-based intervention to help them understand and take ownership of their care. Ourinformation prescriptionsare a new tool that enables effective interventions in a matter of minutes. Currently over 1,200 people a month are using the information prescriptions as a gateway to find support and information about their condition.
