Our Prevention Postcards are a simple way for healthcare professionals to signpost those at risk of Type 2 diabetes straight to information and support about the realities of having the condition. The booklet (PDF 194 Kb) patients will receive explains what Type 2 diabetes is and makes clear the risk factors. It also contains advice on how to make simple, lasting changes to reduce their risk.
