Dr Pokrajac is a Consultant Diabetologist and a clinical lead for Diabetes and Chronic Kidney Disease (CKD) in West Hertfordshire since 2012. She reformed DM and CKD service at Watford and improved management, outcomes and the experience of care for patients with DM and CKD locally. She aims to roll out the reform across West Herts.
Dr Pokrajac is a committee member of Association of British Clinical Diabetologists (ABCD) and the national guidelines writing group on DM and CKD. She initiated a national project on often underappreciated medication-related mortality in DM and CKD with colleagues from Public Health and nephrology. She hopes to secure the funding for the project.
In 2013, Dr Pokrajac co-founded the Herts Diabetes Conference, an annual innovative educational and networking event for all stake holders in Diabetes in the County. In 2014, stakeholders identified inequalities in the quality of diabetes care in West Hertfordshire. In 2015, Dr Pokrajac would like to lead on an independent project of patient-informed diabetes service efficiency and experience improvement for the CCG to tackle the inequalities by increasing the uptake of eight care processes in Diabetes using information technology.
