Who we are
We exist to provide in-depth support for diabetes improvement programmes in the NHS. If you’re a commissioner or a healthcare professional thinking about what you can do to improve care, then you’re in the right place.
It’s our mission to offer hands-on practical support to enable you to create the diabetes services and levels of care your local population needs. We always work in partnership with you to ensure the work we do together is sustainable in the longer term.
We understand how diabetes affects people’s lives and local health and social care systems, and therefore tailor our approach based on your population and needs. Our first step is to work with you to clarify your expectations and agree your success criteria.
From this we can design the details of your programme of support, no matter how big or small. We always evaluate the impact of our interventions so that the effect of any change can be measured and benefits can be replicated and shared.
We have developed a set of tools to drive improvement across key diabetes care areas. These include foot health, prescribing in Type 2 diabetes, use of Patient Activation Measures and the impact of structured education.
Work with us
We have worked with CCGs all over the country to help drive forward improvements in diabetes care. Specifically we believe that primary care provides the greatest opportunity for long-term benefits to the NHS and people living with diabetes.
