The forms can be used to assist school in implementing the medical conditions policy. This section includes a comprehensive Healthcare Plan helping school to record important information on pupils’ medical conditions. The forms include: Healthcare Plan, Template letter, Medication permission and record forms, Staff training record, Residential visits, Risk Assessment Tool for Minimising Triggers, Contacting Emergency Services. These may be adapted to meet the needs of schools.
The forms are based on the DfES ‘Managing Medicines in schools and early years settings’ but have been updated to incorporate advice and comments from school staff, parents and different charities.

Summary:
Guidelines on ‘supporting the administration of insulin in schools’ were produced together by diabetes paediatric staff, school nurses and the LA and sent to all schools in Nottinghamshire and Nottingham City. To support the implementation of this, local experts offer training and awareness sessions for school staff in order to support schools in complying with the guidelines issued by the LA.
