15 Healthcare Essentials

The 15 healthcare essentials are the basic annual health checks that everyone with diabetes should receive. For the vast majority of the time, people with diabetes are responsible for managing their diabetes themselves. Having the checks and having a chance to discuss the results with your doctor or nurse is one of the most important meetings people with diabetes have with their healthcare team.

Unfortunately there is high variability in accessing these annual checks for people with diabetes in different parts of Wales but with your help we can make a difference.

How you can help:

  1. Download and send our template letter (available in right hand column) to your Assembly Member which includes a graph highlighting low performing GP surgeries in your Local Health Board (A map of the Local Health Boards in Wales is below). The letter asks your AM to write to the Local Health Board asking what they plan to do to improve low performance. All you need to do is add your name and the date.
  2. Attend your local Patient Reference Group and/or your Diabetes Planning and Delivery Group. Both Groups allow patients to feedback concerns and best practice directly to the NHS, which encourages patient input as part of diabetes services.
