Delivering diabetes care in a neighbourhood health service

Working with people living with diabetes and their healthcare professionals, we have published our report ‘Delivering diabetes care in a neighbourhood health service (PDF, 4,300KB)’. This report sets out why and how diabetes care should be central to the shift in the NHS to a neighbourhood health service.

One of the key elements to improving the NHS in the Government’s ‘Fit for the Future: The 10 Year Health Plan for England’ published in July 2025, is the plan to prevent and treat more long term conditions outside of hospital and closer to where people live via a neighbourhood health service. 

We want to see this plan result in more people with diabetes receiving support that works for them and their needs and less people experiencing unnecessary complications. Read our response to the Government's 10 Year Health Plan.

Our new report sets out key principles and recommendations for local health systems on how this can be achieved, including with better use of integrated data, targeted commissioning, and innovative models of care.

If done the right way, neighbourhood health services have the potential to ensure more people living with diabetes get the support and outcomes they deserve.
