This conference has been specially designed for those who are finding it increasingly challenging to engage young people in their diabetes care during the transition processes. By taking one day out to share your experiences and learn from others, you will return to your role with the knowledge and motivation to work collaboratively to empower young people with their diabetes care. Featuring:
Developing a network approach: getting it right for young people by working collaboratively across your MDT and understanding how you can adapt ‘Ready Steady Go’ for your service
Improving engagement: maximise motivation to keep your patients engaged, empower young people in their diabetes care and hear lessons learnt from patient experiences
Managing transition on pumps: collaborate with adult services to fully support young people on their pump journey
Working with your MDT: ensure continuity of care, close the gap between adult and child services and improve your transition processes to enhance young people’s experiences
