The Royal College of General Practitioners (RCGP) has launched a 3 minute video animation about care planning. The video shows a typical GP consultation with an elderly patient with multiple and complex illnesses, and recommends care planning as a means of giving the necessary care outside of the constraints of GP consultation times. The video aims to show how care planning works for those working in primary care.
Care planning is a long term and holistic approach to delivering care. It aims to develop a plan to effectively deliver a patient’s long term care, based upon their individual circumstances, and to help people take more control over their own health.
The launch of the video coincides with the launch of the RCGP’s Collaborative Care and Support Planning programme, aiming to encourage the adoption of care planning in general practice.
