Senior Diabetes Clinician, Poole Hospital Foundation NHS Trust
Agnes is a Senior Diabetes Clinician at Poole Hospital NHS Foundation Trust. Diabetes has been Agnes’ passion for the last 12 years. In her current workplace she leads on insulin pump therapy, intensive education in Type 1 diabetes and is the regional Genetic Diabetes Nurse.
Agnes hopes as a Diabetes UK Clinical Champion to create opportunities for practice nurses across the county to work closer together and to build effective working relationships that breakdown the barriers, resulting in better care and experience for patients.Â
Working together could help the focus on prevention; keep patients well, educated and engaged, self-managed and receiving care close to home when it is required. She believes that establishing the Diabetes Practice Nurse (DPN) network would help to increase the quality of care of patients with diabetes receive within their GP practices, as a part of an Integrated Care Service.
She eventually hopes that additional funding will enable the project to increase to include all GP localities within West Dorset, which would allow the DPN network to grow. The DPN network would be trained as diabetes care experts and would aim to provide and to ensure accurate care of patients with diabetes.
