Information prescriptions are a tool for healthcare professionals and people with diabetes to work together to understand and improve on their health targets. They are a short (single side of A4), easy to read and clinically accurate resource that helps healthcare professionals and people with diabetes to make decisions together about the treatment and self-management of their health targets.
They include actions to reduce the risk of complications, and a section for setting individualised behavioural change goals, needed to meet these health targets.
The Information Prescriptions are targeted at people with diabetes who have high blood pressure, high cholesterol and/or high HbA1c. We have now developed four new Information Prescriptions that provide information on:
- Contraception and Pregnancy
- Keeping your Kidneys Healthy
- Diabetes and Kidney Disease
- Diabetes and Mood.
We have worked with IT service providers to make it as easy and streamlined as possible for you to use the Information Prescriptions. If you use EMIS or Vision, you can now access them on your IT system. Have a look at a sample of the Information Prescriptions (PDF, 84 KB) and to use them now go to www.diabetes.org.uk/info-p-qa.
For those on Vision and SystmOne, we are working to get these to you very soon.
