Diabetes Specialist Nurse & ThinkGlucose Lead, Abertawe Bro Morgannwg University Health Board
As one of our Clinical Champions, Chris is looking at several improvement projects within her capacity as ThinkGlucose Lead in Swansea Bay to improve patient experience and reduce medication errors. This will include developing a standardised education programme for all healthcare professionals across primary and secondary care.
Chris has worked in a variety of settings before becoming a DSN. Her interest in diabetes developed during her time in primary care when she began seeing diabetes patients. And after attending several accredited courses, she became the diabetes clinic lead. Chris then moved on to working at an integrated care level between primary and secondary care, specialising in diabetes serves. Chris ventured further and became the Diabetes Lead and Lecturer in Education for Health in Warwick, before returning to Wales to take up her current post. Chris is an honorary lecturer for Swansea University.
Her achievements to date include developing standardised education programmes in both primary and secondary care including working in collaboration with Social Services. She was a group member of the public health guideline on Type 2 diabetes: prevention in people at high risk. She has helped develop and standardise pathways of care for quality improvement projects including pocket medic short films on hypo and hyperglycaemia and safe use of insulin in hospitals. Chris is also a founder member of the Welsh Academy for Nursing in Diabetes (WAND) a group passionate about succession planning. Chris is passionate about improving care for people living with diabetes.
