GP and Clinical Lead for Diabetes, Hambleton Richmondshire and Whitby Clinical Commissioning Group (HRW CCG)
As one of our Clinical Champions, Dr Marfleet will be working closely with her consultant clinical leads to create an integrated diabetes workforce between primary and secondary care.
Priorities include community and practice nurse training, as well as developing a core team of specialist nurses working between primary and secondary care. Part of the model will include virtual advice clinics for primary care clinicians to discuss complex cases and management plans with consultants in the community.
Dr Marfleet has been the GP Clinical Lead for diabetes for HRW CCG since February 2017. She also works in acute medicine in a hospital ambulatory care unit as well as in primary care. This clinical experience gives her valuable insight into the pressures facing both primary and secondary care. She is very enthusiastic about building a collaborative culture and joint working between hospital and community teams.
HRW CCG’s ambition is to commission high quality diabetes care that is centred around people living with diabetes and delivered closer to home wherever possible. The CCG has recently commissioned group education courses for people with Type 2 diabetes through a partnership between the local GP Federation and NHS Trusts. Practice nurses from primary care have now been trained as educators to work with hospital based dietitian educators at local venues in the community.
