General Practioner, Macmillan GP, GP Appraiser, Referral Triager
Jaimin has been a GP since 2003 in Croydon. Having worked with Prostate Cancer UK, and recently Macmillan Cancer Care, around quality improvement projects he is excited to now work with Diabetes UK as an agent for change.
Previous projects with the voluntary sector have included working with patients, primary and secondary care health care professionals to manage patients closer to home with tailored resources and education packages.
He has also used benchmarking of data analysis, significant event audits, and electronic decision support tools to facilitate conversations around sharing best practice to reduce variation in care.
As a Diabetes UK Clinical Champion, Jaimin hopes to improve the actual prevalence of diabetes in the hard to reach groups within Croydon and fully utilise the National Diabetes Prevention Programme.
Additionally, he plans to facilitate the training of Health Care professionals in primary care around diabetes by working with community champions the local CCG, intermediate care and local secondary care colleagues.
