Clinical Programme Manager, NHS Herefordshire CCG
As a diabetes Clinical Champion, Jacinta is working in partnership to deliver an integrated diabetes model for primary care networks in Herefordshire. Improving the quality of diabetes care for all residents is a priority, and this is particularly important in residential and care homes where care has to be highly tailored to individuals with complicated needs.
An immediate priority will be the implementation of a diabetes programme to enhance the diabetes knowledge, skills and competencies of care home professionals to maximise self-care and prevent unnecessary admissions.
Jacinta has worked for the NHS for over 30 years and has experience across clinical, managerial and professional roles. Jacinta is currently working in the NHS Herefordshire Clinical Commissioning Group Service Development team where she has helped set up a local diabetes network and supports a number of diabetes pathways and service development opportunities.
Having previously worked as a clinical nurse specialist, Jacinta is passionate about involving people in decisions about their care and maintaining a strong patient voice to influence outcomes. As a parent of a child with Type 1 diabetes, Jacinta has personal experience and practical insights to the challenges in managing diabetes and is committed to improving care and support to empower patients and families.
