Jennifer Hynes

Jennifer Hynes - Clinical Champion 2018-2020

Programme Lead for Structured Education for people living with Type 2 Diabetes, NHS Forth Valley

As one of our Clinical Champions, Jenny is keen to focus on increasing the uptake of structured education by people newly diagnosed with Type 2 diabetes.

In particular, she would like to identify the barriers to attendance that her community currently face, and to develop innovative ways to ensure that more people benefit from self-empowerment and skills development that structured education can offer. The overall aim is to improve health outcomes for those living with Type 2 diabetes in Forth Valley.

Background

Jenny has been Programme Lead for Structured Education, and a Health Improvement dietitian in NHS Forth Valley since 2013. The Health Board covers a wide range of urban and rural populations across 3 Local Authorities in Central Scotland.

As part of the wider Diabetes Service, Jenny manages the delivery of group education for people newly diagnosed with Type 2 diabetes, and delivers education and support to primary care colleagues.

The Programme began in 2013 and has become established as a valued service, contributing to improved outcomes for those who attend.
