Diabetes Specialist Pharmacist, Northern Health and Social Care Trust
As one of our Clinical Champions, Laura will ‘bridge the gap’ between primary and secondary care, by exploring an outreach pharmacist role to interface between primary and secondary care. Laura will strengthen links between community and practice-based pharmacists in order to work collaboratively across primary and secondary care and improve transition of care for people living with diabetes.
Laura is a Diabetes Specialist Pharmacist in Antrim Area Hospital providing inpatient care to adults living with type 1 and type 2 diabetes. Laura has been a pharmacist for almost 9 years where they have exclusively worked in secondary care in multiple different clinical specialisms, and in diabetes for over 3 years.
Laura’s role is to provide inpatient care to patients living with diabetes, ensuring medicines safety and promoting medicines optimisation throughout the patient’s journey, from admission to discharge from hospital. Laura completed a clinical pharmacy postgraduate diploma in 2014, and a non-medical prescribing qualification in 2016.
In 2020, Laura completed a postgraduate masters which evaluated the impact of a pharmacist-led self-administration of insulin scheme. The scheme is currently in the process of being rolled out across the trust.
Within the multidisciplinary diabetes team, Laura led the insulin safety and hypoglycaemia awareness campaigns in recent years and is an active member of the Trust Insulin Safety Group. Laura works collaboratively with other diabetes pharmacists in Northern Ireland to share resources to ensure a co-ordinated approach to diabetes care within the regional pharmacy teams.
Laura is also regularly involved in pre-registration and postgraduate training of pharmacists and delivers training to the multidisciplinary teams within the trust.
