Integrated service matron and dietitian, Southern Health NHS Foundation Trust
Sarah has worked as a diabetes specialist dietitian for fifteen years across acute and community Trusts in Hampshire, and is currently the Service Manager in the West Hampshire Community Diabetes Service. Being part of the senior management of this dynamic service has provided opportunities to drive improvements, influence diabetes care across the wider system and be part of an innovative team which oozes enthusiasm and passion for diabetes.
As part of this consultant-led specialist community service Sarah delivers group and individual education, manages a clinical caseload and delivers education and support to community and primary care colleagues. The Service began in 2010 and was re-commissioned in 2015 following a competitive tender process. Undertaking the commissioning process and managing a growing diabetic population within a fixed block contract has made her acutely aware of the need to continually evaluate the service to ensure it is relevant, effective, efficient and value for money.
In her role as Clinical Champion Sarah is keen to focus attention on the dietitian’s role and also to consider more patient-led support and activities. In particular she would like to develop a piece of work currently being piloted which includes training lay educators and people with diabetes to provide support and knowledge to people with diabetes. She is keen to better understand whether patient-led education and virtual support groups can improve diabetes outcomes.
