East Lancashire Hospitals NHS Trust

East Lancashire Hospitals Trust (ELHT) are in the process of implementing a new model for the delivery of their diabetes services, integrating acute specialist services with community services on a locality basis. The model is already established in the Hyndburn locality, and ELHT are delivering an improvement programme to rollout the model to the remaining four locality areas.

Review of integrated diabetes services

Diabetes UK worked with ELHT to host engagement events within each locality to allow people with diabetes to influence at an early stage the way that the model will be configured, to allow for local needs. User involvement sessions were held in each of the Ribble Valley, Rossendale and Pendle locality areas. ELHT have now also held the final event in Burnley, and will use the information collated from the four different events to inform the rollout of the integrated services in each of those localities.
