NHS West Hampshire Clinical Commissioning Group

West Hampshire CCG have a number of service improvement projects which they are currently working on. These include a review of the current footcare pathway to reduce fragmentation between services and improve partnership working, and improving the self-management of diabetes patients.

Foot care services and self-management of diabetes

In April and May 2014 Diabetes UK worked with the CCG to host a series of events across West Hampshire to give local people an opportunity to share their experiences of footcare services. We held events in Eastleigh, Brockenhurst and Winchester and across the three sessions over 80 people attended. Attendees were asked to share positive and negative experiences, as well as what their ideal footcare service would look like.Download patient feedback reportand proposals on foot care services (PDF, 223KB)

Following this we arranged further involvement opportunities for local service users, to gain feedback on another of the CCG’s priorities: self-management of diabetes. We held three small focus groups, each for around 10 people and held in Eastleigh, Andover and Totton. This patient feedback report on self management of diabetes will be available soon.
