Aims

To enhance skills and confidence in implementing and providing personalised care planning for people with long-term conditions.

Venue

Arranged locally by workshop commissioners.

Length structure

A 1.5 day course, days separated by several weeks to allow for implementation of action plans developed on day 1.

Cost to student

£2,000 + VAT. Includes planning, preparation and delivery of a course for up to 20 participants from practices, groups of practices, specialist teams or across a locality, eg CCG.

Type of study

Face to face course days, with team coursework in between. Supporting resources provided for preparation and team coursework, including a workshop website and a copy per team of the handbook ‘Personalised Care Planning for Diabetes and Other Long-Term Conditions.’

Entry criteria

Health professionals and other staff (eg administrative) involved in organising and providing services for people with long-term conditions, particularly (but not exclusively) in a primary care setting.

Places available

20 per workshop.

How achievement is assessed

Evaluation on the day of the workshop processes and learning outcomes. Success in achieving personal action plans created as part of the workshop.
