How our Patient Information Packs, in partnership with Fitbit, are supporting people with type 2 diabetes

Patient Information Packs

A diabetes diagnosis is life-changing. It can be a huge shock, and it can be hard to know where to start or how to manage your diabetes. This is why getting someone the right information at the right time is so important. It can help someone feel more in control of their diagnosis and have a better understanding of the impact of diabetes on their daily lives.  

Our specially tailored Patient Information Packs are designed to support someone immediately after a diagnosis. These packs include vital printed information and resources, alongside signposting into our online services and helpline support.  

Working alongside regional health teams and pharmacies across England we can ensure that people receive personalised, local information about type 2 diabetes when they need it most.  

Working in partnership with Fitbit, we want to support people living with and affected by diabetes to have access to the best information and support to manage their health. This is why, with Fitbit’s funding support, we are producing 25,000 Patient Information Packs, free of charge.

Every pack will be distributed by regional health teams specific Integrated Care Systems in designated priority areas in England, where our support is desperately needed. These include Wigan and surrounding areas in the North, and Thanet and the surrounding areas in the Southeast.  

Every pack includes:  

  • Your guide to type 2 diabetes 
  • Information about our Learning Zone platform  
  • Tailored information about local support services.    

By working with regional health teams, we can further adapt and personalise each pack with additional information, such as our foot care or eating well booklets.  

Our Patient Information Packs are printed to be accessible for anyone newly diagnosed, their carers and families.  

Rosanne Dunkley, Specialist Community Diabetes Dietitian, said: 

“The Diabetes UK Patient Information Packs enable us to provide consistent information to all newly diagnosed type 2 patients in the county regardless of which surgery they attend or which locality they live in.

"We also promote our local education pathway via the packs given out by primary care which helps encourage uptake of diabetes structured education."
