Application process

Completing your application form

We have tried to keep our application form as simple as possible. These notes are to help you complete the form.

What you say on the application form is all that will be used to decide whether or not to shortlist you for the post so you must complete all the parts on the form which apply to you. CVs will not be accepted unless specified.

Education and qualifications

Give a list of all the education and training you have undertaken. Please list all professional qualifications you have acquired. If a particular qualification is essential for the job you may be asked to provide original certificates.

Employment history

Starting with your current or last employer list all the employers you have worked for over the past 10 years. Please include periods of paid and voluntary work.

Supporting Statement

Please support your application form with a short resume of your qualities, skills, experience and achievements particularly as they relate to the job description and person specification.


Please give details of two people (not related to you) who can give you a reference. One should be your present or most recent employer and the other should be a previous employer or should know you in connection with your work.

If you do not wish us to contact your referees before the interview we will not do so. However, reference will be taken up on offer of employment.

Equality Monitoring Form

In order to assess how successful our end policy is, we have set up a system of monitoring all job applications. We have asked for your name to enable us to monitor applications at shortlisting and appointment as well as application stage. Your application will not, however, be affected by the information you provide. This form is separate from your application form, is stored separately and used only to provide statistics for monitoring purposes.

Please complete your application form and the equality monitoring form online at Select the appropriate vacancy and click ‘Apply For Job’ Please ensure you send your form in good time to reach us by the closing date. Late applications cannot be considered. This is our preferred method of application but if you do not have access to a computer or the internet you may return your form by post to Human Resources Team, Diabetes UK, 126 Back Church Lane, London E1 1FH.
